Every applicant will receive an email confirming CA's receipt of the application.
Acceptance letters will be sent after the Admissions Committee's review of all applicants.

Forms may be uploaded with application, emailed, mailed, or brought to the corresponding school office during office hours.

List of Required Forms:
- Previous school records. New and transferring students need to make a request to their previous school to release their records to Collegedale Academy when requested.
- Birth Certificate (for Preschool - Grade 8) (Non-US Birth Certificates must be translated in English and notarized)

Submit prior to or within 30 days of your student's first day of school to secure your child's enrollment as required by the state of TN. - Religious Immunization Exemption Form if applicable.
- Physical Form. Submit from the choices below:
- Physical Form** (cannot be more than 12 months old) Required for all new students, if not using an annual sports physical.
- Sports Physical Form** You will need a sports physical each year for CA's Athletics' team sports. (Not for elementary intramurals)
- Three Recommendation Forms (for new students applying to grades 6-12) The link to the online form should be sent to a current teacher, principal or school official, and pastor or church leader. Non-family members please.
*IMMUNIZATION FORM: Families moving to the area from out of state must have their child's immunization record transferred to the state of Tennessee. This can be completed by any Tennessee health department.
**PHYSICALS: Only one is required for admission. Complete the sports physical if you think sports (basketball, cross country, golf, soccer, tennis, track/field, and/or volleyball) could be part of your student's activities for the school year.

Collegedale Academy Admissions Process and Terms
1. Pre-Enrollment Process – The process of following the steps on the “Pre-Enrollment Checklist” to apply and submit all required paper work and records needed.
2. Apply/Applications – An annual process for all students interested in being a student at Collegedale Academy. A completed application holds a student’s spot, but does not guarantee acceptance.
3. Acceptance – Each student is re-accepted or accepted to CA each year once the admissions committee(s) review each application. Each student receives an acceptance letter each year. Even with an acceptance letter a student’s finalized acceptance is contingent on a financial plan.
4. Registration – The event/process in the summer (shortly before school starts) where accepted students go through an “admissions review” to ensure we have all paper work/records, tuition payments and other ancillary information for specific classes.
5. Orientation – A group event typically held for new CA students/families to familiarize them with the building and school schedule prior to or right after school starts, and after registration.
6. Enrollment/Enrolled – When a student has completed the registration process and begins attending classes at CA.

Online Application for Enrollment
New students:
- Begin CA's online application.
- Register for a new account (or log in if previously created).
- Verify account via verification email. If you do not receive this email in your Inbox please check your spam folders.
- Log in and complete application.
Returning Students:
- After selecting "Apply Here" Log In with the user name and password previously created for CA's application.
- Your application information will populate from last year.
- Check and edit each page for accuracy as you go; update with new information.
- If you cannot remember your password, click "forgot password" for help.
Please note: If your student is a senior, please insert his/her social security number. This will be required for the TN Promise/Hope Scholarships verification and dual enrollment classes with Southern Adventist University.
*Prior to the applicant being accepted, the Advisory Council must receive all forms which are included on the online application.
